JOB DESCRIPTIONS:
Our client is urgently looking for the qualified Social Worker to start working ASAP. Faircape requires the services of reputable Social Worker. You may apply before deadline in case you are interested and have required qualifications as short-listed below.
REQUIRED QUALIFICATIONS:
The applicant wishing to apply for this job must meet the following qualifications;
- Must have current SACSSP registration.
- Must have professional indemnity insurance.
- Must have minimum of 3 years’ social work experience.
REQUIRED SKILLS AND COMPETENCIES:
The applicant must have the following skills and competences
- Must have experience in hospital social work and case management.
- Training and experience in palliative care.
- Must have strong report writing and time management skills.
- Must have ability to work collaboratively within a multidisciplinary team.
- Must have willingness to travel for outpatient consultations.
DUTIES AND RESPONSIBILITIES:
The successful candidate will have to implement the following duties but not limited to:
- To conduct psychosocial assessments and contribute to discharge planning.
- To provide counselling, emotional support and mediation for patients and their families.
- To coordinate multidisciplinary care and facilitate referrals to appropriate community and healthcare services.
- To identify and address barriers to recovery, treatment adherence and successful discharge.
- To maintain accurate clinical documentation and participate in multidisciplinary team discussions.
- To advocate for patients’ social, emotional and practical needs throughout their care journey.
